IHC Partnership for Patients Reporting Database

This is the official reporting database for the hospitals participating in the IHC Hospital Engagement Network (HEN).  Hospitals will report process and outcome metrics monthly through the database.  As it has been stated before, the hospitals in the IHC HEN will track their measures for in house ONLY to drive process improvement. This information will not be used for comparable public reporting. Each hospital has  identified a point of contact for completing the reporting of the HEN. If you would like to know who your hospital contact is, please contact us and we can provide you with that information. The following links contain spreadsheets outlining the core, enhanced and high performing hospital best practices with supporting material included for each of the PfP-HEN focus areas.
 
Core Best Practice Resources:
The Core Best Practice Resources are a compilation of evidence-based best practices. This directory is not all inclusive and is not intended as a prescriptive, ready-to-use program of care, but rather as a resource for guiding your hospital's quality improvement efforts as you begin to build your patient safety culture.
 
Enhanced Best Practice Resources:
The Enhanced Best Practice Resources are for those hospitals that have implemented a core safety methodology for a particular HAC, and are now ready to boost their efforts and enhance the organization's patient safety methods. This directory is not all inclusive and is not intended as a prescriptive, ready-to-use program of care.
 
High-Performing Hospital (HPH) Best Practice Resources:

HPH Best Practice Resources are those organizations that have implemented best practices to build a patient safety program, but are ready to further their knowledge of process and analysis. This directory is not all inclusive and is not intended as a prescriptive, ready-to-use program of care.

IHC HEN Metrics Dashboard
Througout the HEN, hospitals will input monthly process and outcome measures to the HEN Portal.  This data will only be seen and used by that hospital to lead their improvement efforts. This data will not be seen by the public or other organizations and will not be publicly reported.

Hospital Work Plan Template
The hospital work plan captures the hospital's plan of action and team members for each clinical focus area.  This template will allow you to input content, which will later be captured on the web-based Reporting Database. Below is the description of the work plan content.

IHI Improvement Map
