The Iowa Healthcare Collaborative tracks hospital involvement in the 5 Million Lives Campaign utilizing an online survey. A secured link, allows providers to share the progress they have made with each of the six projects.

Selections include No Activity, Discussion Only, Partial Implementation, Selective Implementation, or Hospital-wide Implementation.
