Executive Summary
Hospitals and physicians across Iowa are participating in a national project aimed at saving 100,000 lives through reduced mortality by June 14, 2006. The Iowa Healthcare Collaborative is coordinating Iowa’s response with 6 initiatives involving 79 Iowa hospitals. Through conferences, meetings, and site visits, providers are sharing best practice and are working to reduce preventable deaths in Iowa. This work will be the focus of the Third Annual Conference of the Iowa Healthcare Collaborative in November of 2006. Hospitals participating in the six initiatives of this project are listed.
Introduction
The 100,000 Lives Campaign is an initiative of the Institute for Healthcare Improvement to radically reduce morbidity and mortality in American health care. Building on the successful work of health care providers all over the world, proven best practices will be introduced across the country to extend or save lives. This is the first national effort to promote saving a specified number of lives by a certain date (June 14, 2006) and is endorsed by the American Medical Association (AMA), Agency for Healthcare Research and Quality (AHRQ), Centers for Medicare and Medicaid Services (CMS), National Patient Safety Foundation (NPSF) and the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
This is an initiative to engage U.S. hospitals in a commitment to implement changes proven to improve patient care and prevent avoidable deaths. The program begins with these six interventions:
- Deploy Rapid Response Teams …at the first sign of patient decline
- Deliver Reliable, Evidence-Based Care for Acute Myocardial Infarction …to prevent deaths from heart attack
- Prevent Adverse Drug Events (ADEs) …by implementing medication reconciliation
- Prevent Central Line Infections …by implementing a series of interdependent, scientifically grounded steps called the "Central Line Bundle"
- Prevent Surgical Site Infections …by reliably delivering the correct perioperative care
- Prevent Ventilator-Associated Pneumonia …by implementing a series of interdependent, scientifically grounded steps called the "Ventilator Bundle"
In addition to these six interventions, IHI will continuously seek and add others that have been shown to save lives.
The Iowa Healthcare Collaborative is serving as the coordinating organization for the Iowa response to the 100,000 Lives Campaign. To date, 83 of Iowa’s 116 hospitals are participating.

The Hospital Quality Initiative is a voluntary reporting effort by hospitals on a selected group of clinical measures.
The AHRQ database is comprised of administrative data voluntarily reported since the 1990s.
The National Quality Forum (NQF) identified the set of 30 safe practices in May of 2004.
Through conferences, meetings, and site visits, providers are sharing best practice and are working to reduce preventable deaths in Iowa.
