Remember: Your Medication Matters Card is a two-sided document, so make sure you print the two pages included in the PDF on one piece of paper (front and back).
When unfolded, the card's dimensions are 7.75" x 6.75". In order to fit into the plastic sleeve, the card must be folded several times so that the final size is 2" x 3.375".
Once folded, your personal health record information should be visible through the clear plastic sleeve. Don’t forget to fill out both sides of your Medication Matters Card!
