Adverse drug events (ADEs) can happen anywhere. Preventing harm from medications is a top patient safety priority not only in hospitals but also across the continuum of care. The good news is that a large majority of ADEs are preventable. This session will look at how utilizing medication reconciliation can prevent adverse drug events at an institution. Additionally, case-based scenarios will be shared regarding medication reconciliation best practices and engaging the patient and family in the transition of care.
