Community + Clinical Care (C3) Initiatives
Iowa Healthcare Collaborative (IHC) led the State Innovation Model (SIM) in Iowa, alongside the Iowa Department of Public Health and the Iowa Medicaid Enterprise, from 2015 to 2019. SIM was a Center for Medicare and Medicaid Innovation (CMMI) Center initiative focused on aligning payers in value-based purchasing (VBP) and equipping providers for service delivery transformation.
Through SIM, IHC supported communities in the development of Community and Clinical Care (C3) initiatives. C3s are multi-sector groups of stakeholders implementing innovative strategies and referral processes that address social determinants of health needs while applying sustainable population-based, community-applied interventions. 24 C3 initiatives across the state participated in SIM and continue to drive transformation efforts within their communities.
