Statewide healthcare transformation is achieved with focused attention to improve care quality in an environment where clinical and community partnerships lend to healthier communities via a system that is affordable and sustainable over time.
Community and Clinical Care (C3) initiatives are multi-sector groups of stakeholders implementing innovative strategies and referral processes addressing social determinants of health needs while applying sustainable population-based, community-applied interventions. 24 C3 initiatives across the state are driving transformation efforts.
IHC provides executive leadership and delivers a broad array of resources and technical assistance including:

IHC led the State Innovation Model (SIM) in Iowa, alongside the Iowa Department of Public Health and the Iowa Medicaid Enterprise, from 2015 to 2019. SIM was a Center for Medicare & Medicaid Innovation (CMMI) Center initiative focused on aligning payers in value-based purchasing (VBP) and equipping providers for service delivery transformation.
IHC is committed to continuing this work and is diligently advancing these efforts across communities throughout the state.
