Over the past four years, IHC has led the State Innovation Model (SIM) in Iowa, a Center for Medicare and Medicaid Innovation (CMMI) Center initiative focused on aligning payers in value-based purchasing (VBP) and equipping providers for service delivery transformation. Statewide healthcare transformation is achieved with focused attention on the SIM Triple Aim: to improve care quality in an environment where clinical and community partnerships lend to healthier communities via a system that is affordable and sustainable over time.
Community and Clinical Care (C3) initiatives are multi-sector groups of stakeholders implementing innovative strategies and referral processes addressing social determinants of health needs while applying sustainable population-based, community-applied interventions. Twenty-two C3 initiatives across the state are driving transformation efforts.
IHC provided executive leadership and delivered a broad array of resources and technical assistance during the SIM grant including:

- Quality Improvement Engagement and Execution
- Professional Development Training and Consultation
- Community Integration and Partner Alignment
